Health Promotion Center

 

 

In Monterey County, the population is experiencing an 11-percent diabetes rate and a combined overweight/obesity rate in tandem with a parallel rise in healthcare costs.  When an estimated 60 percent of premature mortality can be attributed to lifestyle causes of chronic disease, we recognize the importance of prevention.

Natividad Medical Center has extensive experience in providing patient education and outreach through its Diabetes Education Center. Yet we are now poised to broaden the scope of our programs to develop a Health Promotion Center (HPC) that will focus on education, prevention and wellness.
 
This program will provide Monterey County with the only health promotion center focused on the needs of the safety-net population, with emphasis on overweight or obesity patient education, disease prevention, chronic disease management, and healthy lifestyle choices and practices.
             
The HPC will collaborate with other safety-net providers by offering to provide prevention and chronic disease management services and care coordination, including case management and referrals to appropriate health homes.
 
By centering health promotion in the community, Natividad’s HPC will provide a Center of Excellence for evidence-based interventions provided by local, multidisciplinary expert teams, with the aims of promoting wellness, preventing disease in healthy and at-risk people, and providing chronic disease management for people with the most common conditions affecting the safety-net community.
