Chesney Hoagland-FuchsDiabetes Education Coordinator, Natividad Medical Center Chesney Hoagland-Fuchs, Certified Diabetes Educator, is Natividad's Diabetes Education Coordinator, providing assessments and education to all diabetic patients at the Hospital. At Natividad she created and developed the Diabetes Education Program, specially designed for the diabetes epidemic among Latinos in Monterey County. She has cultivated Diabetes Support Groups for Adults and Teens in both English and Spanish to address proper nutrition, management and care for diabetes patients and their families. In 2007, Ms. Hoagland-Fuchs was nominated for the Robert Wood Johnson Foundation Community Health Leaders Award. Within the community, Ms. Hoagland is well known and participates in numerous collaborations, schools, churches, and local organizations such as the Regional Diabetes Collaborative, and the California Diabetes Program Smoking and Diabetes Leadership Team. Ms. Hoagland-Fuchs earned a Bachelor of Science in Nursing from Georgetown University. She holds a California Registered Nurse License, is Vice President of the California Central Coast Association of Diabetes Educators and is a member of the American Association of Diabetes Educators. |
